Before your first visit.

You may download the Adult Headache Questionnaire and complete it before your first visit. Please be sure to bring it with you. For children and teenagers (up to age 18), please download the Pediatrics Headache Questionnaire.

If you prefer to complete this in the office, please come 45 minutes before your scheduled appointment.

One of the most important parts of your history is a list of the medicines you take, and an accurate list of medications you have taken in the past for your headaches. Please be as complete as possible. Please bring all of the medicines you currently take with you to your first visit.

Please download the headache calendar and start to keep track of your headache activity. Read the instructions and look at the sample calendar. If you're not quite sure how to fill it out, we'll go over it at your first visit.
